Access to Care Crisis
Across Ontario, millions of residents are struggling to find a family doctor or access timely, continuous primary care.
As demand rises and the physician workforce ages, communities face longer waits, greater pressure on emergency departments, and a growing risk of losing access altogether.
The Solution
Hawes Health delivers Hybrid Team-Based Care—giving patients access to a full health team of providers, working together with modern technology to make high-quality primary care more accessible, efficient, and sustainable.
Patients have access to a team of professionals—physicians, nurse practitioners, and pharmacists—so they can get the right care from the right person, faster.
By matching patient needs to the right provider, we reduce wait times, improve outcomes, and keep care convenient and continuous.
Our hybrid model reduces system strain, lowers costs, and creates a stronger foundation for primary care across Ontario.
Hybrid Team-Based Care

Oversee complex care, diagnose, manage chronic conditions and overall health.
Virtual
Provide in-person assessments, follow-ups, health coaching and ongoing care.
In-Person
Manage minor ailments, medications, renewals and conduct medication reviews.
In-Person / Virtual
Easy access to the right care, when and where they need it.

Stronger, healthier communities across Ontario.
Pharmacy-Based Clinics
Our pharmacy-based clinic model expands access by bringing care into communities through local pharmacy partners—making high-quality primary care easier to reach, closer to home, and less dependent on one centralized location.
By spreading care across pharmacy-based locations, patients can access care closer to home with less travel and fewer barriers.
A virtual physician model paired with in-person nurse practitioners brings physician-led care to communities that may not otherwise have access.
Partner pharmacies include private exam rooms and the equipment needed for high-quality in-person visits, with nurse practitioners on site and physicians joining virtually when appropriate.

How Care Is Delivered
Depending on their need, patients are routed to either their family physician or pharmacist for conditions that can be safely treated virtually.
Depending on their needs, patients are routed to their local participating pharmacy to meet with their Nurse Practitioner or Pharmacist, where they can receive full spectrum in-person care in a private fully equipped clinic room.
When appropriate, the physician joins the in-person appointment via live video, assisting with assessments, communication, and treatment.
Physician-led care reaches more communities — without waiting for doctors to move there.
One Connected Platform

Our Technology
Our platform connects patients with the right provider at the right time. From booking and clinical routing to care delivery, prescriptions, follow-up, and reporting, technology supports every step of the Hawes Health hybrid care model.
Smart Routing System
At the time of booking, patients answer a few questions about their symptoms, concerns, and care needs.
Our Smart Routing System uses those answers to determine the most appropriate provider and care setting. It decides whether you should see a physician, nurse practitioner, or pharmacist, and whether your appointment can be virtual or in-person.
This reduces unnecessary appointments, shortens wait times, and lets each provider focus on the patients who need their expertise.
Patients reach the right provider faster — avoiding unnecessary appointments and delays.
How It Works
The patient answers a few focused questions about symptoms and care needs.
The platform assesses the type of care required and whether a hands-on examination is needed.
Virtual consultation, complex care, diagnosis, or chronic-condition management.
In-person examination, assessment, follow-up, and hands-on clinical care.
Minor ailments, medication concerns, renewals, and pharmacist-led treatment.
Advanced Clinical Technology
Every Hawes Health pharmacy-based clinic is equipped with advanced HD mobile cameras and connected digital examination technology.
When a physician joins virtually, the in-person nurse practitioner can position the camera, perform the hands-on examination, and use connected clinical tools so the physician can see, communicate, assess, and treat the patient in real time.
Patients receive the expertise of a physician without sacrificing the quality and clinical value of an in-person examination.

HD Mobile Cameras
High-definition zoom, positioning, and clinical visibility.
Digital Examination Tools
Connected equipment that supports physician assessment from a distance.
Secure Real-Time Video
Private communication between the patient, nurse practitioner, and physician.
On-Site Clinical Assistance
